About Youth Community Living Support Service
The Youth Community Living Support Service team work with young people to set goals for recovery and positive change.
We help youth remain connected to the community and engaged in education and employment to minimise the risk of developing chronic disability requiring frequent hospital admissions, long inpatient stays and long-term care. Assistance for family and carers to build and maintain a positive relationship is also on offer.
Youth Community Living Support Service supports people to:
- Live independently in the community
- Manage mental health and wellbeing
- Improve resilience and social connections
- Develop positive self-identity and self-esteem
- Manage everyday tasks and improve living skills
- Access employment, education, or training
- Sustain accommodation
- Participate in community activities
- Build and strengthen family and friendship networks
- Develop strategies to manage safety and risk.
How to access this service
A referral is required for this service and is made by your clinical care coordinator.
The following people are eligible for Youth Community Living Support Service:
- A person aged between 15 and 24 years (inclusive)
- A person who is clinically managed by a Local Health District community mental health service
- A person who has, or is at risk of developing, significant functional disability
- A person who has provided informed consent to participate in the program.
All referrals are reviewed and considered based on eligibility. If you are eligible, we will meet with you to provide more information about the program, identify areas you would like to work on and discuss how we would work together.
